Before proceeding to complete the reference form below, please ensure that you know the candidate personally or professionally and can confidently vouch for their character, skills, and integrity.

Providing a reference is a significant responsibility. If you do not feel you know the candidate well enough to offer an authentic recommendation, please do not complete this form and ask the candidate to update their reference details.

Also note that family members of candidates or CSKIM team members are not eligible to act as referees.
